• APPLICATION FEE: $50.00
• This form and application fee must be received in the USDF Office on or prior to April 1^st, 2015.
• Applications received after April 1^st, 2015 and April 30^th in the USDF Office will be charged an application fee of $300.
• All applications must be received in the USDF Office with the appropriate fee by April 30^th, 2015. No exceptions.
• Additional horses may be added to an already received application until June 1^st for $50 each.

• Please note that the USEF entry process will be electronic and all required information including FEI Numbers must be effective prior to the date of the nominated entries. NO EXCEPTIONS.
